NOTE about HICAPS: Your Association cannot help you beyond advising you to work with HICAPS and the Health Fund. Your business relationship is between yourself, HICAPS and the Health Fund.
IMPORTANT: When you leave a clinic location with HICAPS facilities you must advise HICAPS that you have left that location. You may do this by filling in their form and sending it to them. If you are uncertain of the HICAPS Terminal Number you will find a Terminal Number on the letter that HICAPS originally sent you when you registered with them. Use this number from the letter. It is important that you personally take this action and do not leave it for any other person to do this for you.
